Likely causes
- Keyset generated before a relevant update. Newer content can expect a generation the keyset does not contain.
- Keyset generated after an update used with older tooling. The reverse mismatch, less common but real.
- An incomplete original. A generation that was never readable during the dump leaves a gap that looks like a revision problem.
How to confirm
Establish two facts: the revision your keyset indicates it contains, and the revision the content or tool expects.
